How to calculate the heart rate from ECG? To calculate the heart rate from ECG, the first thing which we have to do is to find that whether the rhythm is regular or irregular. Mean that the distance between the two successive R wave is equal or not. If the rhythm is regular, we use a formula 300 divided by number of large boxes between two consecutive R wave. As you guys can see in this ECG, the number of large boxes between these two R wave is 3. So, 300 divided by 3 is equal to 100. So, the heart rate in this ECG is 100 beat per minute. And when the rhythm is irregular, we measure the heart rate by determining the number of R wave in 30 large boxes. When we find the number of R wave in 30 large boxes, we multiply that number with 10. And this will give us the heart rate when the rhythm is irregular.
