Right ventricular enlargement and left ventricular enlargement both cause cardiomegaly, but there are some key features in chest x-ray PA view to identify individual enlargement. First we have to measure is that heart is enlarged or not. To do it, we have to divide cardiac diameter by thoracic diameter. If the ratio is greater than zero 0.5, then it can be said that heart is enlarged. When right ventricle is enlarged, apex is shifted upwards and cardiac shadow becomes boot-shaped. On the other hand, left ventricular enlargement causes rounding of left heart border and a positive Schoo sign is noted.
